With changing personal needs, residents may desire assistance related to financial support programs and community support programs, home healthcare, hospice care, and more. The social worker will help identify appropriate resources based on specific needs.

- Advance Care Planning including Living Will declarations, Healthcare power of attorney and Pennsylvania Orders for Life-Sustaining Treatment
- Home & Community based services such as adult day, home health or hospice
- Financial support programs, such as the Pennsylvania Rent Rebate, Pharmaceutical Assistance Contract for the Elderly (PACE), or Needs Enhancement Tier “PACENET” programs
- Psychiatric/Individual Psychotherapy consultation

Vision Loss Support Group- This group meets every other month on the 4th Thursday at 1:30 in Harvest Room. The group is facilitated by Vision Corps. Please call the Residential Living social worker at 717-381-3533 with any questions.
Men’s Caregivers Support Group- This group is for men who serve as caregivers for their wife/partner either in their home or in a care area at Landis Homes. This group is run by Pastoral Services with support from volunteers and other staff as needed. The group meets on the 3rd Tuesday of each month from 10:00 – 11:00am in the Crossings 2nd Floor Meeting Room. Please call the Residential Living social worker at 717-381-3533 with any questions.
Women’s Caregiver Support Group- The Women’s Caregiver Support group is for women caregivers assisting their husband/partner at home or in a care area at Landis Homes. Parkinson’s Support Group- The Parkinson’s Support Group is for those who have been given a Parkinson’s or related diagnosis and their caregivers. This group is facilitated by social workers. Grief Support Group- The Grief Support Group is co-led by Pastoral Care and Social Services. The meeting is for residents who have lost a spouse or significant other during the past year. This group will start on March 5, 2025 from 1:00-2:30pm. The following meetings will be held the first Wednesday of the month from 3:00-4:30pm in the Crossings 3rd Floor Community Room. Dementia and Other Brain Changes- This group is co-led by the Director of Dementia Friendly Experience and the Assistant Nursing Home Administrator. The meeting is held every 4th Wednesday of the month from 1:00-2:30pm in the Crossings 2nd Floor Meeting Room. Support Groups for the Public
Support Groups provide a setting for residents and family members to discuss feelings, concerns and attitudes during and after personal or family crisis situations. Professionally trained staff facilitate many of these groups.
No Longer Alone Family Support Group- This support group provides a place for those who have a loved one living with mental illness to learn and share their experiences. The group meets the 4th Sunday of every month, 3:00-5:00pm in the Crossings 2nd Floor Meeting Room. For more information, call 717-381-3599.
Lancaster County Chapter of the Hearing Loss Association of America (HLAA-PA – The group is ran by a member of the Hearing Loss Association of America. The meeting is held quarterly starting in March on the second Tuesday from 10:00-11:00 am in the Crossings 2nd Floor Meeting Room.
