What to Look For:

• Wet or recently mopped floors without warning signs
• Spilled drinks, food, or liquids
• Snow, ice, or water tracked in at entrances
• Loose rugs, mats, or curled carpet edges
• Torn carpeting or damaged flooring
• Uneven sidewalks, pavement, or walking surfaces
• Cracked tiles or abrupt flooring transitions
• Unattended clutter, boxes, or supplies in hallways
• Unattended trash, laundry, or equipment left in walkways
• Electrical cords, hoses, or wires crossing paths
• Unsecured oxygen tubing or medical equipment cords
• Poor lighting in hallways, stairwells, or parking areas
• Burned-out light bulbs or blocked light fixtures
• Missing, loose, or damaged handrails
• Stairways with worn or missing non-slip treads
• Items stored on stairs or landings
• Wheelchair footrests, walkers, or mobility devices creating obstacles
• Dining chairs, tables, or furniture protruding into walkways
• Resident belongings creating walkway hazards
• Loose thresholds or doorway transitions
• Water leaks from ceilings, pipes, refrigerators, or ice machines
• Leaves, mulch, gravel, or debris on sidewalks
• Parking lot potholes or uneven asphalt
• Missing caution signs where hazards exist
• Improperly stored equipment or supplies
• …Anything that could cause someone to slip, trip, stumble, or lose their balance
